30歲女疑狂食蠔致病

每10宗膽管炎個案中,只有3宗能夠找出致病原因,當中較多是因膽管受細菌感染而長出膽石及化膿。港安醫院外科顧問醫生楊丕祥表示,臨床較為常見是肝臟及膽管受大腸桿菌或沙門氏菌侵襲,相信與病人曾進食未煮熟或生冷食物有關。

受感染而結石化膿

30多歲的鄭女士,3年前開始常進食生蠔,「每個星期都一定要食幾隻」,但去年一頓晚飯後,「覺得個肚好似有道氣頂住」。由於不適症狀沒有影響日常生活,故她沒有立即求醫,但其後近4個月,腹痛感覺斷續出現,故求診接受磁力共振掃描。檢查後始發現左肝的膽管有結石。她於今年初接受微創手術,切除左肝及膽囊後完全康復。

楊指雖然大部份膽管炎成因不明,有個案是因膽管受細菌感染而長出膽石及化膿,市民避免狂吃生冷或有機會感染的不潔食物,可減低受細菌感染的風險,有助預防膽石形成。他稱,現時超聲波、磁力共振及電腦斷層掃描等檢查,均能確定膽石的數量及大小,「有啲大粒嘅(膽石),(直徑)可以有5厘米」,足以完全塞住膽管,故恒常身體檢查亦重要。
